Win a $100 gift card simply by showing you have completed your preventative screenings. Nutranext is giving out three $100 Amazon gift cards to remote employees.  Any remote employee can participate in the raffle by showing a receipt for a preventative care appointment. The contest will end May 31st, and the raffle winners will be picked by June 4th. 

You may enter into the raffle by providing the following (Please do NOT share your prevenative care results):

1) Show a receipt for your primary preventative screening

2) Show a receipt for all gender specific preventative screenings. (mammograms, paps, PSA, colonoscopy)

3) Show a receipt for a biometric screening (you can access this free screening by following the below instructions.)

You will receive one raffle entry for each preventative screening you submit.

Please make sure that participants refer to the company how the name is listed on the agreement. If participants use “nicknames” or alternate company names, it may cause delay in the call center representative locating the company record.
